Transition Coordination and Planning

Special Education Transition Planning and Coordination

In accordance with Senate Bill 943 and House Bill 1554, Franklin County Public Schools
is providing the contact information for the individuals responsible for special education
transition planning and coordination at Franklin County High School.

Transition planning helps students with disabilities prepare for life after high school.
This planning focuses on the student’s individual strengths, interests, preferences, and
goals and may include preparation for postsecondary education or training,
employment, independent living, and participation in the community.

Transition coordination involves collaborating with students, families, school staff, and
appropriate community or adult service agencies to identify, connect, and coordinate the
supports and services needed to help students achieve their postsecondary goals.

In Virginia, transition planning is incorporated into the student’s Individualized Education
Program (IEP) no later than the first IEP in effect when the student turns 14, or earlier
when determined appropriate by the IEP team. Transition services and goals are
reviewed and updated annually as part of the IEP process.
